Responding to increased demand for public access to detailed physician information, the Federation of State Medical Boards has made publicly available a Physician Profile from its comprehensive, nationally-consolidated data bank of U.S. licensed physicians. The FSMB Physician Profile provides users with professional information on physicians and physician assistants licensed in the United States, and includes information on disciplinary sanctions, education, medical specialty, licensure history and locations.

To obtain a copy of your physician's FSMB Physician Profile, please select one of the Order Options at the left. If you elect to submit your request online, your FSMB Physician Profile will be immediately displayed on your screen.
